What makes a distractor work

A good distractor is something a partially-prepared student would genuinely choose. That usually means one of five things: a near-synonym that differs in one important respect, a statement true under a different condition, a claim that is partly right, a correct fact applied to the wrong case, or a negation the reader skims past.

Around 30% of generated questions are built on one of those patterns deliberately. The rest are straightforward recall, because a test made only of traps measures reading care rather than knowledge.

For anyone writing assessment rather than revising, the explanation field is the review tool. If the explanation cannot articulate why a distractor is wrong, the item is not defensible and should be regenerated.
